Our friendly office staff, led by Office Manager Laura Tessitore, work hard to insure that your office visit is a positive experience.
To schedule an appointment please call our office at (631) 689-2600. Plan to spend a few moments on the phone with our receptionist as she takes down some important patient information.
Some health plans require a referral from your primary care physician in order to be evaluated by a specialist. Please come for your consultation with an appropriate referral. In some cases, referrals are made electronically, and need to be in the health plan’s computer prior to your visit.
Please call our office on the day before your appointment to ensure that we have received your referral in order to avoid rescheduling.
If you are unable to keep an office appointment, we kindly ask that you provide us with at least 48 hours notice. This courtesy on your part will make it possible to give your time to another patient. If you wish to cancel a colonoscopy or surgery kindly do so by calling our office. Please do not cancel by speaking with hospital or laboratory personnel only.
Co-payments are due at the time services are rendered. Forms of payment accepted by our office are cash, check, and all major credit and debit cards.
Please have all pertinent insurance information available for your first visit.
A detailed medical history is required for all new patients. Please complete our New Patient Medical History Form and bring it with you on your first visit.
